Alamifovir is a purine nucleotide analogue for hepatitis B virus research

**Background**

Hepatitis B virus (HBV) infection is a major global health burden, often leading to chronic liver disease, cirrhosis, and hepatocellular carcinoma. The management of chronic HBV infection focuses on suppressing viral replication to prevent disease progression. While nucleoside and nucleotide analogues have become the cornerstone of therapy, the emergence of drug-resistant strains, particularly those resistant to lamivudine, poses a significant challenge to clinical efficacy. Consequently, there is a critical need for novel antiviral agents that maintain potency against both wild-type and mutant HBV strains. In this context, we will introduce a purine nucleotide analogue proagent – Alamifovir.

**Definition**

Alamifovir (also known as LY582563 or MCC-478) is a purine nucleotide analogue proagent that exhibits potent activity against the replication of wild-type and lamivudine-resistant hepatitis B virus.

**In Vitro and In Vivo Studies**

According to the Alamifovir description, this compound serves as a potent inhibitor of HBV replication with a sustained antiviral effect. In terms of Alamifovir in vitro activity, studies using MCC-478 (0-10 μM) demonstrated significant potency across various viral strains. The EC50 value for wild-type HBV was found to be 0.027 μM. Furthermore, the compound remained effective against lamivudine-resistant mutants, with EC50 values of 2.6 μM for the M552I mutant, 3.3 μM for the M552V mutant, and 2.0 μM for the L528M/M552V double mutant. Regarding Alamifovir in vivo application, clinical evaluations have assessed the safety and efficacy of the drug in patients suffering from chronic hepatitis B virus infection, confirming its potential as a therapeutic intervention. In conclusion, Alamifovir is a potent purine nucleotide analogue effective against a broad range of HBV strains, including those with acquired resistance.
